Client Rights & Confidentiality
Client Rights & Responsibilities: What are your rights and responsibilities as a client of
Mid-Columbia Center for Living? This document explains what to expect.
Notice of Privacy Practice: This notice describes how medical information about you may be
used and disclosed. It also explains how you can get access to your information.
Release of Information: This form allows you to authorize the release of your health
information. This may be necessary if, for example, your service provider needs to exchange
confidential information with your medical doctor. Your provider will let you know if this is
needed. Read the Notice of Privacy Practice above for more information.
Complaint and Grievance Info: If you are dissatisfied in any way with service you have
received at Mid-Columbia Center for Living, you may file a complaint. If you are on Oregon
Health Plan and are in disagreement with a denial of authorization for a service, you may
file a grievance. This document explains the difference between complaints and grievances,
and the process for filing. If you have questions about this process, you may send e-mail to
complaints@mccfl.org.
Complaint form: Download or print a complaint form. Mail completed forms to Mid-Columbia
Center for Living, 1610 Woods Court, Hood River, OR 97031, Attn: Program Manager. This form
may also be used for grievances. See "Complaint and Grievance Info" above for more information.
